TRIBAL PLAN FOR MY FUTURE

An Oklahoma Successful Adulthood (OKSA) Guide

The Plan for My Future is developed with the youth and with the help of supportive people and permanent connections identified by the youth. It is designed to assist the youth with a successful transition to adulthood. The Plan for My Future should be completed within 60 days of a youth’s 14th birthday or within 60 days of the youth entering care, if the youth is over the age of 14.

COMPLETE THE PLAN

· Prepare for the planning meeting with the youth.

o Complete the Life Skills Assessment with the youth.

o Discuss the purpose of the planning meeting and the 7 Key Elements for Success.

o Determine a location, date, and time for the meeting.

o Invite supportive people and permanent connections identified by the youth.

· Meet with the youth and their permanent connections.

o Discuss the purpose of the planning meeting.

o Review the Life Skills Assessment Score Report that was emailed to you after completion of the Life Skills Assessment.

o Provide the youth with a copy of the Plan for My Future found at http://www.oksa.ou.edu/sites/default/files/pdf/ilplan.pdfor provided in the assessment results e-mail you received after the youth completed the Life Skills Assessment.

o Use the instructions on the Plan for My Future document to begin the plan with the youth.

o Use the youth’s Plan for My Future during the development of the 90 Day Transition Plan (My Transition Plan), which should be developed 90 days prior to the youth’s 18th birthday.

DOCUMENT THE PLAN

· Scan the completed Plan for My Future and email it to OKSA@ou.edu, please include the youth’s name and date of birth in the body of the email, or fax a copy of the plan (include youth’s name and date of birth) to the National Resource Center for Youth Services (NRCYS) at (918) 660-3785, ATTN: OKSA Program Specialist.

 WORK THE PLAN

· Prior to each monthly visit, print or download the youth’s Plan for My Future, and take the copy with you.

· Review the plan with the youth during your monthly visit.

o Update goals as needed.

o Update and end date steps as needed.

o Create new steps as needed.

o Ask the placement provider to assist the youth in completing new tasks each month.

o Access Youth Development Funds to support the youth’s plan.

o Provide updated copies of the plan to youth, placement providers, NRCYS, etc. after each visit.
