We need your help! Be on the lookout for a survey in the mail from OKSA when you turn 17 and possibly when you're 19 and 21. Be a voice for foster youth throughout our state—complete the survey and return it to us as soon as possible.

NYTD stands for the National Youth in Transition Database. NYTD collects information to see if Independent Living (IL) services are helping youth successfully leave foster care.

Oklahoma is gathering information through a short survey of select youth (it depends on your birthday) at ages 17, 19, and 21, and by keeping track of what services youth receive between the ages of 16-21.

Be a Foster Care Advocate

Completing this survey will help other foster youth by letting us know what we are doing right in the OKSA program.

By completing the survey, YOU help Oklahoma continue to receive federal funding!

You will receive an incentive for completing the survey.
